The Health and Wellness Section provides health education to inform and empower citizens to make healthy lifestyle decisions. Topics include:

  1. Chronic Disease Management

    This chronic disease program supports strategies to improve multiple chronic diseases and the risk factors associated with certain public health priorities.

  2. Community Health Assessment

    The Shelby County Health Department conducts a community health assessment to determine strategic health priorities for Shelby County. The Department uses a process called M.A.P.P (Mobilizing Action for Planning and Partnership) to guide this process.

  3. Health Promotion

    Health Promotion focuses not only on health education but disease prevention as well. We promote the empowerment of Shelby County residents by sharing information for improved healthy lifestyle choices.

  4. Tobacco Prevention

    The Tobacco Risk Reduction Program offers information and education on the prevention of tobacco use.
